How to use this site.

(Please read and follow these basic steps.)

  1. Click on the form above you wish to use.

  2. Input the information from the participants information sheet.

  3. Click the "Proofread Information" button at the bottom of the page.

  4. Proofread the information entered.
  5. If all the information is correct, click the "Submit Information" button.
  6. If you find it necessary to correct information for one of your participants after the forms have been entered, click the "Correct Registration, Wrap-Up or Reunion Survey Form link.  Then select the proper entry.
  7. Click HERE if you need to find your class dates.
